✓3. Right route Same-day care, routine appointment, pharmacy, minor illness hub or another appropriate service.

Current service notice: the practice posted on 25 August 2026 that it is temporarily unable to provide ear syringing because of a shortage of disposable equipment. Ear checks can still be offered when needed before an audiology appointment, but patients requiring syringing may currently need to have this done elsewhere.

Start here: match your task to the right Elms route

The fastest route depends on whether you need clinical advice, a repeat medicine, a test result, a home visit or administrative help.

You need Start here What happens next
Life-threatening emergency Call 999 Do not wait for the GP online form or a callback.
GP appointment or medical query Use the official Contact us online form between 8:00am and 5:30pm Monday–Friday. A GP reviews the request on the same working day.
Urgent issue after 5:30pm but before the practice closes Call reception. Reception can route urgent issues while the practice is still open.
You cannot use the online form Call 01582 767444. A receptionist can complete the online request form for you.
Home visit Call before 10:00am where possible. A GP phones first to assess the request before confirming a visit.
Repeat prescription Use NHS App or the practice’s admin-query route. Allow at least five working days.
Blood, urine or sputum result Check NHS App after 7–10 days. A GP will already have reviewed the result before it appears in the app.
New-patient registration Complete the NHS-linked GMS1 registration route. The practice checks whether your address is inside its Harpenden-area catchment.
Urgent help when closed NHS 111 Use 999 instead for a life-threatening emergency.

If you need a broader explanation of NHS GP appointments, registration and out-of-hours care, see the MedicalPracticeUK NHS medical-practice guide. The Elms’ own current instructions should take priority for live patient requests.

Opening hours and extended appointments

Day Core surgery / reception hours Additional appointment access
Monday 8:00am–6:30pm Pre-booked GP appointments at The Elms from 6:30pm–7:45pm.
Tuesday 8:00am–6:30pm Other PCN extended-access options may be available.
Wednesday 8:00am–6:30pm Other PCN extended-access options may be available.
Thursday 8:00am–6:30pm Other PCN extended-access options may be available.
Friday 8:00am–6:30pm Pre-booked GP appointments at The Elms from 7:00am–8:00am.
Saturday Closed Pre-booked PCN care may be available elsewhere depending on service.
Sunday Closed Use NHS 111 for urgent non-emergency help.
Extended hours are not normal reception hours. The practice says reception operates with skeleton staffing during its Monday evening and Friday early-morning sessions, so routine reception queries and general appointment booking cannot be handled in those periods.

Appointments: every request is reviewed by a GP

The Elms does not simply place every patient into the next available GP slot. Its current model asks patients to describe the problem first so a GP can decide whether the safest next step is same-day care, routine care, pharmacy support, a minor-illness hub or another service.

1
Submit Contact us online between 8:00am and 5:30pm.
Use the green online-contact route on the practice website for an appointment request or GP query.
2
Explain the main issue clearly.
Give enough information for the reviewing GP to understand what is happening and how urgent it may be.
3
Keep your phone with you.
The practice warns that it may contact you within minutes and may need a prompt response.
4
Follow the route offered.
A medically urgent request may lead to a same-day appointment; a routine issue may be booked later; some problems may be directed to a community pharmacist or another local service.

What might happen after GP review?

Same-day Elms appointment

Urgent problems may be offered an appointment at The Elms on the same day.

St Albans Minor Illness Hub

The practice says some same-day problems may be offered care at the Minor Illness Hub at St Albans City Hospital.

Community pharmacy

Where appropriate, the reviewing GP may direct a patient to a community pharmacist rather than a GP appointment.

Routine appointment or advice

The practice may arrange a routine appointment or provide a non-urgent GP response without a separate consultation.

No internet access? Call 01582 767444. The practice explicitly says a receptionist can complete the same online request form on your behalf.

Appointment length and practical rules

Appointments are typically around 10 minutes and the practice says it can usually address one main problem per appointment. If you need an interpreter attending in person, tell reception when booking because the practice says it may need up to two weeks to arrange one. A telephone interpreter can also be organised.

A chaperone can be requested when booking, at check-in or directly from the clinician. If you cannot attend, call reception on 01582 767444 and choose option 2, or reply to the appointment reminder text to cancel.

Home visits: phone before 10:00am where possible

The Elms reserves home visits for patients who are housebound. If you believe a home visit is needed, call early rather than submitting a routine request and waiting.

1
Call before 10:00am if possible.
Explain why the patient cannot reasonably attend the practice.
2
Expect a GP telephone assessment.
A GP will call to gather more information before confirming whether a visit is needed.
3
The visiting clinician may not be a GP.
The practice lists GPs, physician associates, paramedics, advanced nurse practitioners and GP trainees among staff who may undertake visits.
If the clinical situation appears to require emergency treatment, the practice may advise calling 999 rather than waiting for a home visit.

Repeat prescriptions: allow at least five working days

The practice currently asks patients to allow at least five working days for repeat-prescription processing and does not accept routine repeat requests by telephone.

Prescription route How to use it
NHS App Select the repeat medicine from your NHS account and submit the request.
Admin online query Use Contact us online, choose the admin-query route and follow the prompts.
Paper request Use the right-hand section of a previous prescription or write the request and place it in the red prescription box at reception.
Pharmacy assistance Some pharmacists may be able to request repeat medication on your behalf.
Telephone ordering The practice says routine repeat-prescription requests cannot be taken by telephone.

Check your nominated pharmacy

Most prescriptions are sent electronically using EPS. Make sure your nominated pharmacy is correct before your prescription is issued. The practice cannot send an electronic prescription to a pharmacy outside England.

Medication review or monitoring overdue?

Long-term medicines may require annual review, blood tests, blood pressure, pulse or weight monitoring. The practice warns that repeat medicines may not be issued when required monitoring is overdue.

For broader repeat-medicine troubleshooting, see the MedicalPracticeUK medication and prescription guide.

Test results: check every result rather than assume silence means normal

The Elms gives unusually clear advice about following up investigations. The practice says patients should actively check that requested results have returned and that any required follow-up is in place.

Investigation Published Elms guidance
Blood, urine or sputum Usually viewable in the NHS App after 7–10 days.
Ultrasound or X-ray May take longer than routine laboratory tests.
NHS App result The practice says a GP will have reviewed it before it appears and may attach advice or an explanation.
No NHS App access Submit an admin online consult or call reception after 2:00pm.
Important: the practice says it is possible for individual test results to go missing somewhere in the pathway, so patients should check that every result has returned when several tests were requested.

Where to have an Elms blood test

The correct blood-test location depends on who ordered the investigation. This is one area where using the wrong site can waste a journey.

Elms-requested blood test

The practice says patients can use Harpenden Memorial Hospital (Red House) or a West Herts hospital blood-test site.

For Harpenden Memorial Hospital, The Elms books the appointment. Call reception or submit an admin online consultation to receive a booking link.

There is no walk-in service at the Harpenden Memorial Hospital route described by the practice. Patients without an appointment may be turned away.

Hospital specialist requested the test

The Elms says the test should normally be taken at the hospital where the specialist is treating you, because the specialist’s team is responsible for receiving and interpreting the result.

Do not assume a hospital-requested test should be redirected to your GP practice.

Register with The Elms Medical Practice

NHS.uk currently shows Elms Medical Practice as accepting new patients, but the practice says it can only accept people who live within its practice area covering Harpenden and some surrounding villages.

1
Confirm that your address is in the practice area.
The admin team checks eligibility after the registration form is submitted and may contact you if more information is needed.
2
Complete the NHS-linked GMS1 registration form.
The official Elms registration page links directly to the NHS registration route.
3
Provide a daytime telephone number.
The practice asks for a suitable contact number in case its team needs to clarify registration information.
4
Watch for the health questionnaire text.
The Elms says it does not routinely send a separate confirmation once registration is complete, but it will text a link to a basic health questionnaire while full records are transferring.
Medical records can arrive in stages. The practice says some electronic history from a previous English NHS GP may arrive within days, while older paper records can take several weeks.

Temporary patient registration

People staying locally for a period may be able to register temporarily for up to three months while remaining registered with their permanent GP. Contact the practice on 01582 767444 if this applies to you.

Clinics and local services that may avoid a standard GP appointment

The Elms has a multidisciplinary team and several health needs are handled through nurses, clinical pharmacists, physiotherapists, mental-health practitioners or community pharmacy rather than a GP.

Long-term condition reviews

The practice runs structured reviews for chronic conditions, with nurses and doctors involved according to the condition and monitoring needed.

Diabetes

Patients may be asked to complete blood and urine tests before an annual diabetes review, with the subsequent clinic determined by the result.

Minor illness

The Elms has clinicians trained in minor illness, and some urgent cases may also be directed to the St Albans City Hospital Minor Illness Hub.

Pharmacy First

The practice promotes community pharmacy assessment for several common conditions, including sore throat, sinusitis, infected insect bites, impetigo, shingles and some uncomplicated UTIs.

Harpenden Health PCN Wellbeing Hub

The local PCN wellbeing hub supports adults with early-stage mental-health difficulties and brings together Harpenden GP practices with local voluntary-sector partners.

Ear care notice

As of the practice’s 25 August 2026 notice, ear checks remain available but ear syringing itself is temporarily unavailable because of equipment supply issues.

Accessibility, parking and arrival at Stewart Road

Wheelchair and mobility access

  • Designated disabled parking is listed close to the entrance.
  • A permanent access ramp and wide front door are provided.
  • Patient services are on the ground floor.
  • The reception desk includes a wheelchair-height section.
  • An accessible toilet is available.
  • The practice keeps a wheelchair on site for patients who need assistance.

Hearing and communication

  • A hearing loop is listed at the practice.
  • Telephone interpreter support can be arranged.
  • An in-person interpreter may need up to two weeks’ notice.
  • The practice also describes communication support for British Sign Language users.

Parking

The practice facilities page lists 29 parking spaces plus separate GP parking, with marked disabled spaces at the side of the building. There is also limited public parking on Stewart Road. If parking is essential for a mobility need, confirm current arrangements before travelling.

Waiting-room alternatives

The practice says patients who find waiting rooms difficult—for example because of neurodivergence or immunosuppression— can ask to wait in their car and provide a mobile number so the clinician can call them when ready. Electronic and QR-code check-in are also available.

Hospital referrals: what happens after the GP refers you

Most specialist referrals require GP assessment first, but The Elms also publishes several local services that accept self-referral.

NHS specialist referral

The practice says patients referred through NHS e-Referral should receive a letter within around two weeks with instructions for booking and any password or reference information required.

Self-referral

The current referral page lists self-referral options for areas including anxiety, depression, drugs and alcohol, sexual health, smoking cessation, stress, weight management and young-carer support.

Complaints, suggestions and Patient Participation Group

Formal complaint

The 2026 complaints procedure asks patients to raise a complaint as soon as possible, normally within 12 months of the incident or within 12 months of discovering the problem.

Complaints can be sent to elmsmedical@nhs.net with the message directed to the Practice Manager, or raised through the other official routes listed in the procedure.

Patient Participation Group

The Elms PPG provides feedback on practice services and patient communication. The practice says adults who have joined since November 2022 are automatically members unless they opt out.

Elms Patient Participation Group
The general suggestions form is not intended for medical queries or formal complaints. Use the medical online-consultation route for a health concern and the formal complaints route for a complaint.
