SystmConnect micro-guide: choose the right request
| Need | Use this SystmConnect route | Important detail |
|---|---|---|
| New or urgent health problem | Clinical/health problem request. | Submit one request per person. Include symptoms, start date, red flags and best contact number. |
| Routine health request | Clinical request. | Practice says it responds within 6–8 hours during opening times for appointment requests. |
| Routine admin | Admin/routine care request. | Allow 24–48 hours before chasing routine/admin requests. |

| Test result question | Query form. | Official results page says response within 24–48 hours during opening times. |
| Prescription query | Prescription/query form or phone after 10am. | Do not use urgent appointment route just to chase routine repeat medicine. |
Repeat prescriptions: online, post box, pharmacy nomination and 2–3 working days
| Prescription task | What to do | Avoid this mistake |
|---|---|---|
| Order repeat medicine | Use NHS account/NHS App, SystmOnline, repeat prescription request form, or bring/post the paper form to the locked prescriptions box at any of the four surgeries. | Do not phone/email routine repeat requests unless a vulnerable-patient prior arrangement exists. |
| Collection timing | Prescription is usually collectable from pharmacy 2–3 working days after ordering. | Do not wait until you have no tablets left. |
| Choose pharmacy | Nominate/change pharmacy in the app/website, at the GP practice, or at a pharmacy that accepts repeat prescriptions. | Do not assume your old pharmacy is still nominated after moving area. |
| Prescription query | Ask local pharmacist for medicine advice, or phone Care Navigation after 10am. They can raise query to pharmacy team. | Do not use emergency appointment route for routine medicine questions. |
| Medication review | Attend when the practice asks you to come in for review. | Ignoring review requests can delay repeat medicine. |
| Old medicines | Return old medicine to pharmacy or bring it to surgery. | Do not put medicines in household bin or flush them. |
Test results: NHS App, phone/visit 10am–4pm, and query form
| Question | Practical answer | Best action |
|---|---|---|
| Where can I see results? | Once reviewed by a doctor, results can be viewed in NHS account/NHS App. | Check NHS App first if you have access. |
| Can I phone? | Phone or visit between 10am and 4pm. | Have name, date of birth, test type and test date ready. |
| What if result needs action? | The practice says it will contact you if results show you need more tests or treatment. | Keep phone details up to date. |
| Need to discuss result? | Fill in SystmConnect query form from Monday to Friday from 7am; response usually 24–48 hours during opening times. | Include who requested the test. |
| Hospital-requested test? | The hospital/clinic that ordered it is usually best placed to explain it. | Contact that department first if the GP did not request it. |
I am asking about a [blood/urine/x-ray/swab/other] result from [date]. It was requested by [GP/hospital/clinic]. I still have [symptoms] / I need to know whether any action is needed. My best contact number is [number].
Sick / fit note route: new note vs extension
| Situation | Correct route | Useful wording |
|---|---|---|

| New sick note | Clinical SystmConnect request. | Give exact start date, condition, job demands and whether you can do adjusted duties. |
| Extend existing sick note | Admin SystmConnect query form. | Include previous fit-note end date and new date needed. |
| Phone/visit alternative | Call or visit any of the four surgeries Monday–Friday between 8.15am and 6pm. | Bring employer deadline and previous note if available. |
