How Charlton’s GP appointment system works

Need a GP on the day?

Call 01952 620138 and choose option 2. Charlton currently uses a combined on-the-day system.

1
Phone on a day when you are available.
The practice says patients who are not available to attend that day will normally be asked to call back on a day when they can attend.
2
You may first be offered a telephone advice call.
This lets the GP decide whether the problem can be managed remotely or needs examination.
3
If the GP decides you need face-to-face assessment, it is arranged for the same day.
4
Expect one main problem in a standard appointment.
Charlton states that ordinary GP appointments are usually around 10 minutes and intended for one problem.

Online same-day advice calls

Patient Access releases a limited number of online advice-call appointments after midnight. The practice says these remain available until 8:30am or until booked, whichever happens first.

Book ahead instead

Pre-bookable face-to-face GP appointments can be available up to two weeks in advance. You can check Patient Access or call option 2. Charlton advises avoiding the busiest telephone periods around 8:30am and 2:00pm for advance-booking enquiries; the reception desk can also check availability after 10:30am.

Extended access: evenings and every fourth Saturday

Charlton says it now offers extended-access appointments on Monday and Friday evenings and every fourth Saturday, subject to availability, in coordination with the Wrekin Primary Care Network.

What extended access is useful for

Working-hour conflicts

An evening or occasional Saturday slot can be useful if normal daytime attendance is difficult.

Planned care

The practice’s patient information highlights uses such as health checks, cervical screening and mental-health support where the appropriate appointment is available.

It is not a Saturday walk-in service. Ask reception to book an available extended-access slot and confirm where and when you should attend.

You may be offered a same-day community-pharmacy consultation

Charlton participates in the Community Pharmacy Consultation Service. If reception decides that your symptoms are suitable for community-pharmacy assessment, you can be referred for a same-day booked consultation at a pharmacy.

This route is intended to help patients with suitable minor illnesses receive care quickly while keeping GP appointments available for more complex problems.

This is a clinical route, not reception “sending you away”. Community pharmacists are qualified healthcare professionals and can assess many minor conditions, give treatment advice and refer back to another NHS service if the problem needs different care.

Repeat prescriptions: POD has closed and processing now takes four working days

The local Prescription Ordering Direct (POD) service stopped being available for Charlton prescription requests from 28 November 2025. Patients should now use one of the practice’s own published routes.

Ordering route How to use it
NHS App / NHS account This is the practice’s preferred route. Select the repeat medicine shown in your account.
Patient Access Choose medicine already listed as an authorised repeat item.
Practice medication form Use the repeat-medication request form on the official website.
Email Send the medication request to charlton.online@nhs.net.
Written request Put your written repeat request into the prescription post box in the surgery foyer.
Telephone Routine repeat-prescription requests are not accepted by telephone.
Allow four working days. Charlton specifically asks patients not to wait until medication has run out. It says prescriptions can be ordered up to seven days before they are needed.
Ordered before 3pm Practice says ready after 4pm
MondayFriday
TuesdayFollowing Monday
WednesdayFollowing Tuesday
ThursdayFollowing Wednesday
FridayFollowing Thursday

Charlton uses the Electronic Prescription Service, so most approved prescriptions can be sent directly to a nominated pharmacy.

ADHD and antipsychotic shared care: the practice currently states that it does not accept shared-care agreements for ADHD medication or antipsychotic medication. Patients affected by this should clarify prescribing responsibility with the specialist and practice rather than assuming Charlton will take over prescribing.

Test results: check online or call between 3pm and 5:30pm

If a result shows that you need more tests or treatment, Charlton says the practice will contact you. Once a doctor has reviewed a result, it can also be visible through your NHS account or Patient Access.

1
Wait until the result has had time to return and be reviewed.
2
Check NHS App/NHS account or Patient Access first.
Reviewed results may be visible without a telephone call.
3
For a blood-test result by phone, wait at least five days.
Charlton asks patients not to telephone earlier than five days after the blood sample was taken.
4
Call reception between 3:00pm and 5:30pm.
This is the practice’s published result-query window.

Sick notes: self-certify for the first seven days

7 days or less

Your employer should normally use self-certification rather than asking you for medical evidence from the GP.

More than 7 days

Charlton asks patients who need their first fit note after more than seven days of sickness to arrange an appointment with a doctor.

If Charlton has already issued a fit note and you need an extension, you can submit a new online request or ask reception by phone or in person.

No emergency slot is needed just because the certificate expires. Charlton says fit notes can be backdated where clinically appropriate, so an emergency GP appointment should not be used solely to renew a certificate.

Home visits: call before 10:45am

Requests for a GP home visit should be made by calling 01952 620138 before 10:45am.

Charlton limits home visits to patients who are housebound or too ill to attend the surgery. Reception will ask for information about the problem so the doctor can assess urgency.

The practice asks patients to attend the medical centre whenever they reasonably can because examination facilities are better and tests can be organised more easily there.

Register with Charlton Medical Practice

The practice provides a postcode catchment checker, online new-patient registration form and paper registration forms at reception.

1
Check your postcode against the Charlton catchment area.
Do this before completing the full registration form.
2
Choose online or paper registration.
Complete both the registration information and health questionnaire if using the online route.
3
Include your NHS number if you know it.
The practice says this is helpful when matching your NHS record.
4
ID and proof of address are helpful but not a condition of registration.
Charlton explicitly says that being unable to provide identification documents does not prevent you from registering.
No fixed address? Charlton identifies itself as a Safe Surgery and states that people who are homeless or have no fixed abode can still register. A temporary address, day-centre address or the practice address may be used where appropriate.

Temporary patient

If you are staying locally for up to three months, temporary registration may be possible while you remain registered with your normal GP elsewhere. Contact reception because a practice does not have to accept every temporary-registration request.

Medication warning: Charlton currently states that it does not accept shared-care agreements for ADHD medication or antipsychotic medication. If you take one of these medicines, clarify prescribing responsibility before changing GP.

Clinics and healthcare professionals available through Charlton

The practice publishes both traditional GP/nurse clinics and access to a wider healthcare team. Reception may ask a few confidential questions so you can be directed to the professional best suited to the problem.

Long-term conditions

Published essential services include asthma, COPD, coronary heart disease and diabetes care.

Women’s health

Additional services include cervical screening, contraceptive care and maternity services.

Children

The practice lists child health surveillance and vaccination/immunisation services.

Minor surgery

Minor-surgery provision is included in the practice’s published service list.

Minor injuries

Charlton lists selected minor injuries such as grazes, bruises, minor lacerations, sprains, puncture wounds and non-life-threatening allergic reactions.

Community pharmacy

Suitable minor illnesses can be routed to a same-day community-pharmacy consultation rather than waiting for a GP.

Parking and disabled access

Charlton describes its Lion Street building as a modern GP premises with free parking and disabled access.

Free parking

Parking is available at the medical centre. Allow extra time at busy periods rather than assuming a space will always be immediately available.

Disabled access

The practice specifically publishes disabled access as a building feature. It has also previously acted on patient feedback by repainting car-park markings to protect access to disabled parking.

Need a specific adjustment? Call reception before travelling if you require mobility assistance, communication support, an interpreter or another reasonable adjustment. General “disabled access” wording does not tell you whether every individual facility will meet your particular need.

Complaints: use the practice manager or current NHS STW route

Charlton’s complaints page was updated in August 2026 and separates informal feedback from a formal complaint. The general contact form is not intended for medical matters or formal complaints.

1
For a formal complaint to Charlton, use the complaint form or ask reception for the complaints procedure.
Explain what happened, when it happened and what outcome you want.
2
If you do not want to complain directly to the practice, contact NHS Shropshire, Telford and Wrekin Patient Services.
3
Current local NHS contact:
Telephone 01952 580407 or email stw.patientservices@nhs.net.
4
For primary-care complaints, the ICB explains that the formal investigation may be coordinated through the West Midlands hub.
The local Patient Services team can route the complaint correctly.
Time limit: NHS Shropshire, Telford and Wrekin says a complaint should normally be made within 12 months of becoming aware of the issue, although later complaints may sometimes still be considered.

CQC rating: Good overall, with a 2017 inspection

CQC currently displays Charlton Medical Centre as Good overall. Safe, Effective, Caring, Responsive and Well-led are all rated Good.

CQC event Date / result Context
Overall rating Good Current rating shown on CQC’s live profile.
Comprehensive inspection 20 November 2017 Inspection underlying the latest full report.
Report published 12 January 2018 CQC published the practice as Good overall and Good in all five key areas.
Later data review 6 July 2023 CQC said the information available did not indicate that the rating needed reassessment at that stage.
This is not a new 2026 inspection. CQC still displays the Good rating, but the inspection and review dates should remain visible so patients understand the age of the underlying assessment.

When Charlton Medical Practice is closed
