Goldsworth Medical Practice: GP triage, two Woking sites, appointments and hours
Goldsworth Medical Practice provides NHS GP care across Goldsworth Park Health Centre and York House Medical Centre in Woking. GP appointment requests are clinically triaged rather than simply placed into the next free slot, while nurse and healthcare-assistant appointments can be booked directly by phone.
The practice currently publishes the same main telephone number for Goldsworth Park Health Centre and York House Medical Centre. Use it if you cannot use the online form, need a nurse/HCA appointment, need help with registration or results, or have an access or communication need.

GP appointments: how Goldsworth triage works
Goldsworth introduced its GP triage model in April 2024. The practice’s update explains that an experienced GP works with administrative staff to review online appointment requests, rather than leaving reception staff to determine clinical urgency.
The form is usually available between 8:00am and 5:00pm Monday to Friday. It can be accessed on the practice website and through the NHS App.
Explain the main symptoms or concern, how long it has been happening and whether you believe you need urgent help. Do not use the Admin Query form for a medical request.
The information is used to decide how quickly you need help, which professional is best placed to help, and whether an in-person consultation is necessary.
The practice contacts patients during surgery hours after the request has been reviewed.
What can happen after triage?
Same-day or next-day care
Where clinically appropriate, an urgent request may be offered an appointment on the same working day or the next day.
Routine appointment
Where appropriate, non-urgent requests can be arranged within the next two weeks.
Different professional
The request may be better handled by a nurse, clinical pharmacist, healthcare assistant or another service.
Hospital / urgent-care advice
If the information suggests immediate hospital care is needed, the practice may direct you to an urgent service rather than booking a routine GP slot.
Nurse and healthcare-assistant appointments do not need GP triage
This is an important difference from GP booking. The practice says appointments with its practice nurses and healthcare assistants can be booked directly by phone.
ECG
The practice lists ECGs among services provided in-house. Book the appropriate clinical appointment rather than submitting a general GP triage request unless advised.
24-hour blood pressure monitoring
Ambulatory blood-pressure monitoring is among the in-house services highlighted by the practice.
Contraception procedures
The practice lists coil fittings and contraceptive implants among its in-house clinical services.
Joint injections
Joint injections are also listed among the services provided by the clinical team. Eligibility depends on clinical assessment.
Cervical screening
Eligible patients can arrange cervical screening when invited. If the practice has asked you to book, NHS App booking may be available.
NHS Health Check
Eligible adults aged 40–74 may be invited for an NHS Health Check, which the practice says normally takes about 30 minutes with a nurse or healthcare assistant.
Home visits
If you cannot leave your home, tell the practice when you request medical help. The current appointment guidance confirms that home-visit needs should be included in the request so the team can decide the most appropriate route.
Repeat prescriptions: use NHS App, online form or written request
Goldsworth does not accept routine repeat-prescription orders by telephone. This is a patient-safety rule published by the practice.
| Route | How it works |
|---|---|
| NHS App / NHS account | Choose the repeat item and submit it to the practice for approval. |
| Practice online form | Use this if you do not have access to the NHS App or another online prescription service; provide the medicine name and dose. |
| Written repeat slip | Place the repeat slip in the prescription drop box at the surgery. |
| Telephone | Routine repeat-prescription orders are not accepted by phone. |
| Nominated pharmacy | Approved electronic prescriptions can be sent directly to your nominated pharmacy. |
Medication reviews
If you take regular medicine, the practice recommends checking that treatment remains appropriate at least annually. Ask for a medication review if one is due or if you are experiencing side effects.
The MedicalPracticeUK medication guide gives broader help with repeat medicines, nominated pharmacies, medication reviews and urgent supply problems.
Test results: NHS App, online form or phone after 2pm
Goldsworth provides several ways to check GP-held results including blood tests, urine tests, X-rays and ultrasound results.
If the result is available to you online, this can avoid a phone call.
The practice says it will respond to an online test-result enquiry within five working days.
The practice specifically asks patients to contact the surgery about results after 2pm.
Register with Goldsworth Medical Practice
Start by checking the practice boundary. Goldsworth says patients usually need to live inside its catchment, although temporary registration may be possible if you are in the area for less than three months.
Use the practice-boundary checker on the official registration page. If you are unsure whether your address qualifies, contact the surgery.
The practice asks new patients to make sure they have around one month’s supply because record transfer from the previous GP can take around a month.
The online registration form takes around 15 minutes. Paper registration is also available from the surgery.
The practice says including a copy of identification can help with registration and record matching.
Use Pharmacy First when a GP appointment may not be necessary
Before requesting a GP appointment, Goldsworth specifically asks patients to consider whether a community pharmacy can help more quickly.

Pharmacy First can assess eligible people aged 12 and over.

Eligible patients aged 5 and over may be treated through Pharmacy First.

Feedback, complaints and patient participation
Formal complaint
Goldsworth’s current formal-complaint page says complaints should normally be made within 12 months of the event or of becoming aware of the issue. The practice aims to acknowledge formal complaints within three working days and aims to provide a written response within 25 working days.
If you are complaining for another adult, the practice may need that patient’s written consent before discussing confidential information.
Feedback and PPG
The practice also has a general feedback form and a Patient Participation Group. The PPG is intended for patient views about services and practice development; it is not a route for individual medical questions.
